HealthConnection app for iPhone and iPad

HealthConnection helps clinicians find programs and services for clients with chronic conditions and health needs. HealthConnection contains programs delivered by public and non-profit agencies plus physicians and private home health providers operating on the North Shore of Metro Vancouver. In time HealthConnection will include more resources for different populations on the North Shore.

- Search by keyword, program name or specialty (e.g., cardiology)
- Filter by type of program or service
- View results in map or list view
- View essential information including location, hours of operation, clients served, referral process and fees
- Email links to program descriptions for easy printing or sharing with colleagues or clients
